On October 25th, 2016, the Thunder River Rapids Ride at Dreamworld theme park in Queensland, Australia malfunctioned, resulting in the deaths of four park guests. This event will be examined as a case study in the importance of process safety and risk management. We will closely review the circumstances of this tragic event and use bowtie diagrams to illustrate the key risk management learnings.

Most people familiar with bowtie diagrams understand the main benefits including:

  • hazard identification,
  • hazard communication, and
  • determination of critical controls.

Bowties can also be used for complex applications to improve the development and maintenance of performance standards, procedures, and overall verification of a risk management system, including risk-based decision making.
